GIVE THIS ENTRY FORM TO APPOINTED TOURNAMENT DIRECTOR FROM YOUR CLUB READ ALL RULES!!!!! - CALL OR E-MAIL TOURNAMENT DIRECTOR WITH ANY QUESTIONS ENTRY MUST BE RECEIVED 7 DAYS PRIOR TO EVENT FOR ENTRY FORM MAY BE PHOTOCOPIED

IN SIGNING THIS ENTRY FORM, the undersigned agrees to the following: (i) to release all other contestants, tournament officials, hosts, and sponsors for any and all damages, claims, demands, or expenses relating to or resulting from this tournament, including personal injury and/or property damage, (ii) not to sue or make any claim against any individual or group involved with this tournament now or in the future concerning any injury or damage related to or resulting from this tournament, (iii) to indemnify and hold North Texas Bass Club Challenge and their appointed officers harmless for any liability whatsoever if North Texas Bass Club Challenge or its officers are involved in a lawsuit resulting from the undersigne's actions, regardless whether intentional or accidental, and (iv) to accept the tournament director's decision as final in all matters pertaining to the tournament. This release will also serve as notice that my insurance carrier will not sue or make any claims against North Texas Bass Club Challenge and its officials for any claims they may pay on my behalf. The undersigned HEREBY CERTIFIES BY HIS OR HER SIGNATURE on this form that he or she has read and understands all the rules and regulations concerning this tournament, which are set out on this entry form and the reverse side thereof. Any misstatements or untruthful statements made by the undersigned on this entry form are the undersigned's own personal responsibility and the undersigned agrees to hold North Texas Bass Club Challenge and its officers harmless regarding any claims made as a result of such misstatements or untruthful statements.
